I hereby give permission for my child to participate in Youth Activities held by Calvary Chapel for the calendar year 2026.
I hereby give permission for Calvary Chapel of Bangor’s pastors, elders, leaders, staff, and volunteers to take whatever necessary measures to obtain emergency medical services in the event of an injury to my child.
I hereby give permission for Calvary Chapel of Bangor’s pastors, leaders, staff, and volunteers to act on my behalf for medical emergencies in the event that I cannot be contacted or if a situation arises where there is not time to obtain approval.
I also give permission for my child to be transported by church-owned vehicles, personally-owned vehicles belonging to Calvary Chapel of Bangor employees or volunteers, or rented vehicles.
I agree to hold harmless Calvary Chapel of Bangor and its affiliated organizations, employees, agents, and representatives, including volunteers and other drivers, from any and all claims arising from my child’s participation on this trip.
This release agreement does not apply to claims of intentional (criminal) misconduct or gross negligence by Calvary Chapel of Bangor, its employees, or volunteers. If such circumstances are proved in a court of law, I acknowledge and agree that the church can assume no financial liability beyond its actual liability insurance policy in force.
